If your Trial requires the submission of BCSP Forms, you may do this in the “Digital BCSP Form” Tab:
To begin the BCSP Form Upload, you’ll first have to select the Site, Subject, and Timepoint of the submission the form is in correlation with:
Once this has been completed, click “Next”. You’ll be prompted with a second screen requesting further information about the Subject and Exam the BCSP Form is related to:
On this page, you may choose to go Back one step, completely Reset the Form, or proceed to the Next step once all of the necessary information is provided.
NOTE: BCSP Forms and information required from you may vary depending on the Protocol you’re uploading them for, depending on the Study-specific requirements facilitated by the Clario Study Team.
Providing all the requested information and clicking “Next” will take you to a screen that allows you to review your BCSP form once more before submitting it:
A pop-up requesting your electronic signature will appear after clicking “Submit”. Here, you should enter your login credentials (e-mail and password) and click “Submit” once again. If needed, you may click on the “X” in the upper right corner of the message to close it and check your BCSP Form again:
Submitting the form will take you to the following screen. Please do not close this window:
Once finished, a pop-up will appear at the top of the screen, highlighting a successful Form Upload:
